of the crest/shield/arms/other device:

as an attachment via e-mail a copy of the artwork;
Please fax a copy of the artwork to
us on (01489) 581104;
Please send
a wax impression of a previous engraving to us via snail mail;
Alternatively,
add a brief verbal description in the "Other Details"
box below.
